Recording a Field Experience Entry:

This is the one form you fill in yourself, and the only record you can later change or remove.

Field Experience Timesheet form

  1. Observation Details
    • The form knows who you are from your sign-in, so it opens straight at Observation Details. 
      • This is why you must be signed in as yourself: whoever is signed in is who the entry is filed under. 
    • Enter the Observation Date.
    • Then choose the Course this experience belongs to. 
      • The dropdown lists the courses on record for you; entries read YEAR-TERM-SESSION-COURSE , for example 2024-FA-A-ED520 .
  2. Time Spent
    • Enter the Hours and Minutes for this experience. 
      • Both start at 0. Record the actual time you spent, not a rounded estimate.
  3. What You Did
    • Under Please select all that apply, start with Grade Level(s) — tick every level present in the setting you observed (Preschool, Elementary, Middle, High, and Special Populations). 
    • Then, under Field Experience Activities, tick all the categories that apply: 
      • Observation in schools and related agencies 
      • Student tutoring 
      • Interactions with families of students 
      • Attendance at school board and school-based council meetings 
      • Participation in a school-based PLC 
      • Opportunities to assist teachers 
      • Other 
    • More than one can apply to a single visit. Tick every option that applies. 
    • Between those two groups sits Student Ethnicities Observed 
      • tick the ethnicities represented in the setting you observed. 
      • Along with the grade levels above, this supports the School of Education's accreditation reporting on the diversity of candidate placements, so complete it from what you actually saw.
  4. Reflection
    • Under Reflection / Summary, write your account of the experience. 
    • This is the substance of the entry — what you observed, what you did, what you took away. 
      Note: The box holds up to 1,000 characters and counts down as you type.
  5. Demographics of Educator Observed
    • The last section is about the P-12 partner who hosted you, not about you or the students. 
    • Enter their first and last name, phone, email, school and its address, certification area(s), years of teaching experience, rank, and the grade they teach. 
      • Getting their email write is important, as it is how your experience is tied back to the cooperating teacher's records.
  6. Submit
    • Select the submit button at the bottom. 
    • If anything required is missing, the page comes back with a summary at the top and everything you typed still in place. Fix what it lists and submit again.
